UnitedHealthcare Community Plan Medicaid (MMA)

Our Florida Medicaid Managed Medical Assistance (MMA) plan offers a range of benefits. It’s for pregnant women, children, families and adults who meet income requirements. We offer support to adults with special care needs.

UnitedHealthcare Community Plan Long-Term Care (LTC)

UnitedHealthcare Community Plan Long-Term Care (LTC) is a coordinated care management program for community-based elderly who are at risk of nursing home placement. Get the extra help you need to live independently in your own home for as long as possible.1

UnitedHealthcare Community Plan (Medicaid (MMA) and LTC)

The Florida Medicaid Plans offer a range of benefits along with extras that you may find useful. 

Our Medicaid (MMA) plan covers children, families, pregnant women and single adults who may qualify based on income. UnitedHealthcare Community Plan Long-Term Care (LTC) is a coordinated care management program for community-based elderly who are at risk of nursing home placement.

Dual Special Needs plans

Dual Special Needs plans (also called Dual Health plans or D-SNPs for short) are for people who have both Medicaid and Medicare. Dual plans cover most doctor visits, hospital stays and prescription drugs. They offer many extra benefits beyond those you get with either Original Medicare or Medicaid.

Individual & Family ACA plans

UnitedHealthcare offers affordable Individual & Family ACA plans in Florida. You can choose from 3 plans: bronze, silver and gold. Each plan comes with a variety of benefits and customer support.

The Florida Medicaid plans provide medical coverage at no-or low-cost to families, pregnant women, children, people with disabilities and adults who meet income requirements.

Enrollment

Join or switch to UnitedHealthcare Community Plan of Florida. Open enrollment for Florida Medicaid is available year-round.

  • Transportation to and from medical appointments
  • Prescription drug coverage

Transportation Assistance

Our Florida Medicaid plan provides unlimited round trips to and from health care locations. That includes rides to:

  • Doctor or therapy visits
  • Health departments
  • Vision clinics
  • Urgent care centers
  • Pharmacies

  • Members may earn $20 for joining the 1-hour call

A healthier life is in your future

Being healthy can be hard, but we have programs to support you. Our UnitedHealthcare Healthy Behaviors program can help you:

  • Lose weight
  • Quit smoking
  • Get help for substance abuse
  • Take care of yourself during pregnancy and after having a baby (UnitedHealthcare Healthy First Steps®)

You’ll work with a health coach and your doctor to set goals and track your progress. When you reach certain milestones, you can earn rewards.

GEDworks

The GEDWorks™ program is available for eligible members of UnitedHealthcare Community Plan of Florida. This benefit is:

  • Completely free for all eligible members
  • Available with all the tools you need to pass the GED test
