Hello
Let's dive in
Narrator:
As you consider your health plan options, here are a few great questions to ask.
Screen text:
How to choose a health plan
Screen visuals:
A hot-air balloon drifts upward.
Narrator:
Starting with: is your provider in the network?
Screen text:
Is your provider in the network?
Narrator:
Think about the providers you see regularly, like your primary care provider, also called a PCP. If you’d like to keep seeing them, it’s a good idea to make sure they’re in the plan’s network.
Screen visuals:
A primary care provider listens to a young child’s heart as she sits on her mother’s lap. Then, an adult participates in a virtual visit with a primary care provider on a laptop.
Narrator:
Just so you know, a network is a group of providers and facilities who've been contracted to deliver health care services to you, often at a discount. Staying in your plan’s network when getting care makes sense — because it may help you save money.
Screen text:
A network is:

A cluster of circles appears. In each circle is a health care-related item, such as a heart, prescription bottle and health care provider. One by one, the circles turn into coins. A piggy bank appears, and the coins drop into it.
Narrator:
To check if your PCP or other provider is in the network
Screen text:
How do you check if a provider is in the network?
Narrator:
We first have to ask: Are you in the process of considering a new health plan? Or are you already a health plan member who's registered with a my-u-h-c dot com account? Depending on your answer, the way to find a PCP will be different.
Screen visuals:
As a hand holds a smartphone, a question mark appears on its screen. A second hand holding a smartphone appears. On its screen, a check mark appears.

Narrator:
If you're considering a plan, and want to see if a PCP is in the network, you'll go to the my-u-h-c dot com home page. Select Find a Provider and then choose coverage type. Next, choose type of care. From there, you’ll see additional steps where you may need to set your location to find PCPs near you and select your plan. Then, you’ll choose "Primary Care", and select View All Primary care providers. Providers who are in the network will be marked with a green check.
Screen visuals:
A step-by-step demonstration illustrates how to search for a network primary care provider on myuhc.com.
Narrator:
If you're already a member with a registered account—to find a PCP you'll need to go to my-u-h-c dot com and sign in to your account. Select find care. Then, primary care providers. Providers who are in the network will be marked with a green check.
Screen visuals:

Narrator:
Second question: What are your health care needs? Think about the kind of health care you or your family might need in the upcoming year.
Screen text:
What are your health care needs?
Narrator:
Do you have ongoing health conditions? Or have regular visits to doctors? Are you expecting a big change — like a new baby or a planned surgery?
Screen text:
Consider your care needs
- Major health needs
- Frequent specialist visits
- Plans for future changes

Once you’ve considered your health care needs, you’re ready for question 3: How do you like to manage health care costs?
Screen text:
How do you like to manage your costs?
Narrator:
Some people manage costs by choosing to keep their monthly premium payments low. Others choose to pay higher monthly premiums because it tends to lower other costs, like copays or deductibles.
Screen visuals:
A calendar appears under the words “Monthly premium,” and a wallet is under the words “Copays and deductibles.” Between them is a horizontal line. As the calendar sinks down, the wallet rises. Then, the calendar rises as the wallet sinks.
Narrator:
Either way, it's helpful to compare plan differences in deductibles, copays, coinsurance and out-of-pocket limit amounts. Being aware of the details can help you keep your costs in check — and know what to expect to pay.

Another question to ask is: What kind of benefits and tools are included with your plan?
Screen text:
Are there benefits and tools included with your plan?
Narrator:
It’s good to take into consideration all the different additional benefits — like wellness programs, plus the helpful digital tools — that come with your health plan at no added cost to you.

Health plans are designed to help take good care of you. Knowing how they work can help you get the most out of your benefits. Let’s get into it.
Screen text:
How a health plan works
Screen visuals:
A lotus flower, apple, bandage, digital thermometer, smartwatch, blueberry, drug capsule and medical cross nestle into a neat pile.
Narrator:
Most common health plans have a premium, or plan costs, which is a routine payment typically taken out of your paycheck. This helps keep your plan active, so you can stay covered.
Screen visuals:
A calendar’s pages flip forward, and a check mark appears on the same day of each month. A hand sets an empty drinking glass next to the calendar.
Narrator:
At the start of your plan year, you pay 100% of your covered health services until you meet your deductible, which is the dollar amount set by your health plan before it starts to pay costs.
Screen text:
Your coinsurance may vary by plan or service. This example is for illustrative purposes only. Please refer to your official plan documents for coverage details.
Screen visuals:
A hand pours a pitcher of water into a drinking glass. When the glass if half full, a horizontal line appears, labeled “Deductible.” A second hand holding another pitcher of water appears and begins pouring into the same drinking glass.
Narrator:
Once you reach your deductible, co-insurance begins. That’s the percentage of costs your health plan shares with you. For example, you may pay 20% of a covered medical expense, and your plan will pay the remaining 80%.
Screen text:
Your coinsurance may vary by plan or service. This example is for illustrative purposes only. Please refer to your official plan documents for coverage details.
Screen visuals:
Two streams of water fill a drinking glass. The halfway mark of the glass is labeled “Coinsurance.” Water continues to pour into the glass.
Narrator:
Your plan offers you further protection with an out-of-pocket limit, which is the most you could pay for covered services in a plan year. Typically, coinsurance and deductibles count toward your out-of-pocket limit — but premiums don’t. Once you’ve met the out-of-pocket limit, your plan’s got you covered at 100% when you see network providers for eligible expenses the remainder of the year.
Screen visuals:
A drinking glass is filled nearly to the brim with water and ice cubes. At the top of the glass, a horizontal line appears, labeled “Out-of-pocket limit.” Two streams of water begin to pour into the glass. The water rises up to the horizonal line. The top half of the drinking glass is labeled “Coinsurance and deductibles.” The label at the top of the glass changes to “Health plan pays 100%.”
Narrator:
Now let’s take a closer look at your plan.

Screen visuals:
A young adult meets with a health care provider in the provider’s office. The provider holds a tablet as they review information together and talk.
Narrator:
So it’s good to know that preventive care is covered for zero dollars out-of-pocket by most plans when you see network providers.
Screen visuals:
An apple and a calendar slide into view on a shelf. A check mark appears on the calendar, and the words “$0 out-of-pocket” appear on screen.
Narrator:
Your plan may require prior authorization before you receive certain medical services, treatment plans, prescriptions or durable medical equipment. Generally, if you’re receiving services from network providers, your PCP or provider will obtain the prior authorization for you. But, if you go out-of-network, you may be responsible for obtaining it yourself.
Screen text:
Prior authorization may be required for:
- Certain medical services
- Treatment plans
- Prescriptions
- Durable medical equipment

Understanding your pharmacy benefits

Welcome in. We’ve got lots of good info to share with you — so let’s get started.

Welcome
Let's get started
Narrator:
When it comes to your health, medications can play an important role. So it’s good to be in the know about your pharmacy benefits: what they are, how they work and the ways they can help you save on costs.
Screen visuals:
Illustrations of an eye, apple, heart, lotus flower, bandage and medication pills nestle into a neat pile and then scatter. The capsule pill isolates and moves under a magnifying glass. The magnifying glass swipes away. The capsule pill multiplies, and the pills drop into a prescription medication bottle. The lid of the prescription bottle is secured, and the bottle flips into an outstretched hand.
Narrator:
It all begins with Optum Rx, which is the service that’s used for all things pharmacy. Whether you’re at home or on the go, Optum Rx is designed to help make it easier to manage your medications, and save on them too.
Screen visuals:
A paper map unfolds. A location pin is labeled “Rx” in the center of the map. The words “Optum Rx®” appear, and then a prescription medication bottle appears between the words “at home” and “on the go.” A weekly pill organizer appears. One of the compartment lids pops open, revealing 3 pills inside.
Narrator:
By using our digital tools, the UnitedHealthcare app and my U-H-C dot com, you can find and compare medication costs, locate a network pharmacy nearby, manage your home delivery orders and view lower-cost alternatives through the prescription drug list, or P-D-L.
Screen visuals:
A smartphone and desktop computer appear. They’re replaced by a search bar, where “My Rx” is typed. “Pharmacy 1,” “Pharmacy 2” and “Pharmacy 3” appear with a series of dollar signs next to each listing. A location pin labeled with a prescription pill bottle drops onto a street grid. Above the location pin, a search bar reads “Nearby pharmacy: 1 mile away.” A mailbox appears, and as its door opens, a prescription pill bottle slides out. The words “Prescription drug list (PDL)” appear on screen.
Narrator:
What’s great about the P-D-L is that it gives you a list of commonly prescribed medications and lets you know which ones are covered by your plan. The PDL also arranges medications by tiers, from typically lower-cost generic drugs on Tier 1 to higher-priced brand name ones on Tier 3. You can use the PDL to discuss lower-cost options with your health care provider.
Screen visuals:
On a piece of paper, 2 columns are labeled “Rx” and “$.” A prescription pill bottle slides into place next to the paper. The label of the prescription pill bottle unwraps and shows “Tier 3 $$$,” “Tier 2 $$” and “Tier 1 $.” The pill bottle highlights the words “Tier 2 $$,” then “Tier 1 $,” followed by “Tier 3 $$$.” As it moves back to “Tier 2 $$,” the label wraps back around the bottle, and “Rx” is added to the label.

To help you use the PDL more effectively, there are 3 tips to keep in mind. First: Check the PDL for coverage rules before you fill or refill prescriptions. Some medications may require certain actions first. The next tip is to check the PDL often for tier changes. The PDL is updated throughout the year, which means a medication’s tier or coverage may change. Finally: Check the PDL for a lower-cost option. If you use a medication that’s in a higher tier, ask your provider to check the PDL for a lower-tier option that would work for you that typically costs less.

When it’s time to fill your prescriptions, there’s a couple ways to save on costs. You can choose to pick up the prescription medication at a network pharmacy, which typically costs less than an out-of-network one. Or, you can choose home delivery and have your medication shipped right to you at no additional cost.

Not all prescriptions are eligible for home delivery. You are not required to use home delivery for your prescriptions. There are other pharmacies in your network.
