Short-Term Disability Plan (Not for use in NM and NY)

Video transcript
On a white background, the blue UnitedHealthcare logo appears—the letter U, with its right arm split into three stripes. It ripples across the background like a ribbon and disappears to the right, leaving blue text in the center in its wake. Cheerful mandolin music with hand-clapping plays.
ONSCREEN TEXT: Short-term Disability
NARRATOR: A short-term disability plan is like protection for your paycheck. It’s designed to help replace part of your income in the event a qualifying injury, illness, or health event temporarily prevents you from working.
The text fades, and a cartoon of an orange hand in a bright blue shirtsleeve reaches out from the left. A golden coin drops into the outstretched palm of the figure’s hand. The hand disappears, replaced by a blue IV bag marked with a white cross. The IV bag rolls to the left, and a split view in the center shows a white head against a deep blue background. Inside the head, a blue cloud appears. A tear falls from the white figure’s eye, and a sun peeks out from the cloud inside their head. In the third split view on the right, a woman holds her pregnant belly.
NARRATOR: With short-term disability, you’ll be paid a percentage of your paycheck each week. This way, you’ll still have income as you take time off work to recuperate until you’re able to work again, or until you reach the end of your benefit period—whichever comes first. Now against a white background, a plant and a mug of tea slide onto a shelf. A calendar appears above the shelf, and blue text fades in to the left:
ONSCREEN TEXT: You’re paid each week
Three orange checkmarks appear in three separate squares on the calendar. The view zooms in, transforming one of the calendar squares into a view through a windowpane. Inside, a figure sits with their leg in a cast up on a footstool. The white cast transforms into a white running shoe, and the figure hits the ground running. Blue streaks behind the shoe indicate their speed.
Blue fine print appears at the bottom of the teal background:
ONSCREEN TEXT: The benefit period for your plan is determined by your employer. Please see your Certificate of Coverage for plan details, including your plan’s benefit period.
NARRATOR: Short-term disability is built with an easier claims process in mind. Online, you can manage claims, set up direct deposit, and check your correspondence and payment history24/7. You can also connect by phone or mail.
Now a blue computer monitor slides in from the right. A message slides up onto the screen with white dollar signs and blue lines. Dollar bills fly into the computer screen and transform into orange dots with blue checkmarks inside. A white envelope notification appears on the screen with a blue number 1 in an orange dot. The envelope vanishes, and three checkmarks in orange dots take its place in the center of the screen.
Two speech bubbles—one blue and one white—appear on the teal background. Three white lines pop up in the blue bubble, and a yellow plus sign in the white bubble. Blue fine print appears at the bottom:
ONSCREEN TEXT: Telephonic access not available with all plans.
NARRATOR: Once you submit your claim, you may have to wait a short period of time—also known as the “elimination period”—before you’re eligible to receive your weekly payments. These payments go directly to you and can be used however you want: to put toward your mortgage or rent, medical bills, childcare, or anything else you may need.
A hand inserts a white envelope into a blue mailbox. On a white background, a bird with deep blue feathers sits next to a list with round bullet points:
ONSCREEN TEXT: Submit your claim
Wait a short period of time
Receive your payments
A yellow triangle with an orange exclamation mark pops up over the bird’s head as it tweets. The bird turns its head and blinks, then takes to the air. It soars over a small blue house with a yellow door. Blue text fades in on the left of the house:
ONSCREEN TEXT: Payments go directly to you
The view splits in half, with the house on the right and a folder of documents on the left. A white cross marks the folder and a paperclip secures a photo of a person to the front. The view splits into thirds, and a teddy bear slides onto a shelf on the far left-hand side.
NARRATOR: Also good to know: you may be able to work part-time and still receive short-term disability payments.
A clock appears in a teal square on a blue background, with white text to its left:
ONSCREEN TEXT: You may be able to work part-time
An ID card slides down behind the clock, as if swiping in for a shift at work.
NARRATOR: When you’re ready to get back to work full-time, short-term disability is ready to help set up you up for success by connecting you with a Disability Specialist, who’s trained to help you return to work. If you’re no longer able to perform your essential duties and can’t return to work, a specialist can assist you in finding another job by helping you update your résumé, build skills, prepare for interviews, and more.
Now a blue briefcase appears beside a yellow thermos and an apple. Two speech bubbles pop up with talking heads inside. One of the figures wears a headset, and nods while speaking. A view from above shows a computer on a desk, where a figure moves a wireless mouse with their hand. A clipboard on the desk enlarges into the center of the view, and fills with three blue squares. Checkmarks pop up in the squares as the clipboard slides to the left, and a list with round bullet points pops up on the right:
ONSCREEN TEXT: Update résumé
Build skills
Prepare for interviews
On a white background, a figure’s hand holds up an orange heart on their index finger.
NARRATOR: Short-term disability. Here to help you feel a sense of financial support as you take time to care for your health.
The heart gently sways back and forth. Blue text pops up in the center word-by-word:
ONSCREEN TEXT: Take time to care for your health
New blue text appears:
ONSCREEN TEXT: Learn more at whyuhc.com/financialprotection
The text fades away, and a striped blue ribbon winds its way into the center, forming the UnitedHealthcare logo. It fades into blue text:ONSCREEN TEXT: UnitedHealthcare
The blue text fades away, and black fine print appears at the bottom of the view:
