If it’s part of your plan, you may earn rewards for completing healthy activities. These may include going to see your provider or completing health screenings. Health Perks Rewards

Through the Health Perks™ program, eligible members can earn up to $250 in rewards³ simply by completing a few health and account-related activities, like visiting your primary care provider (PCP) and creating an account online.

One Pass Select

One Pass Select can help you get and stay fit in ways that work for you and your budget. This includes access to thousands of $0 on-demand and livestreaming digital fitness classes and workout videos. You also have the option to buy a discount gym membership at thousands of popular locations, which comes with $0 grocery delivery and more perks.⁴

Walgreens Smart Savings Discount

Eligible members can save 20% with the Walgreens Smart savings card on thousands of eligible Walgreens branded over-the-counter health and wellness products,⁵ including:

Eye care like saline solutions and eye drops
Dental care like mouth guards and dental floss
Baby essentials like wipes and baby thermometers
Smoking cessation products like nicotine gum and patches

Reward offerings may vary by plan and are not available in all plans. By participating in the program or accessing rewards funds, you agree to the Rewards Program Terms of Service. Rewards must be earned and reported within time frames specified by the plan. Time frames are available within the activity details on the rewards program home page.
Member Rewards is not available with all UnitedHealthcare Community plans.
Members age 18 and over can earn up to a $250 reward on a prepaid Visa® card upon completion of eligible activities. To earn $50, complete the following activities: set up a UnitedHealthcare member account on myuhc.com/exchange, opt-in to all types of text messages, sign up for all paperless plan communications, and the subscriber of the plan signs up for Autopay of premiums on behalf of all members enrolled on the plan (not available in New Jersey). To earn $50, complete the health risk assessment available on myuhc.com/exchange (excludes members in a Copay Focus plan in partnership with Kelsey-Seybold Clinic in Texas). To earn $150, complete a primary care provider (PCP) visit. The PCP reward is earned after claim is processed and can take 6-8 weeks. Limit one reward per eligible member.
