UnitedHealthcare Community Plan Long-Term Care (LTC)
Eligibility for enrollment in the UnitedHealthcare Community Plan Long-Term Care (LTC) plan is determined by CARES (Comprehensive Assessment and Review of Long-Term Care Services).

Call and speak to a Medicaid Choice Counselor toll free at 1-877-711-3662, Monday–Thursday, 8 a.m.–8 p.m., and Friday, 8 a.m.–7 p.m.. TTY/TDD users please call 1-866-467-4970.
You can also visit the SMMC website at www.flmedicaidmanagedcare.com to create a Member Portal account or chat with our Enrollment Virtual Assistant.
You will need the Medicaid ID number of each member and the account PIN number.
